Adult deformity reconstruction addresses the full three-dimensional deformity, the sideways curve, the front-to-back imbalance, and the rotation, to restore the ability to stand upright and walk without pain.
Adult spinal deformity reconstruction corrects complex deformities, including degenerative scoliosis, front-to-back imbalance, and deformity after prior surgery, that significantly impair the ability to stand, walk, and function.
Unlike adolescent scoliosis, which mainly addresses the sideways curve, adult deformity reconstruction must also restore front-to-back balance, the alignment of the spine over the pelvis. A patient who cannot stand upright without bending the knees to compensate has positive imbalance, one of the most disabling deformities in spine surgery and the main driver of pain and limitation.
Reconstruction usually involves multilevel fusion with screws, osteotomies to correct rigid deformity, and fixation to the pelvis to anchor the construct and prevent failure at the base. These are among the longest, most demanding procedures in spine surgery, and Dr. Enguidanos’s fellowship training at the University of Colorado prepared him specifically for this work.
Reconstruction is reserved for deformity that significantly limits standing, walking, and quality of life. Where appropriate, physical therapy, medication, and injections are tried first.
When imbalance or progressive deformity continues to cause pain and disability despite conservative care, reconstruction is the appropriate path to restore alignment.
Detailed planning begins with full-length standing X-rays and measurement of the pelvic parameters that determine how much curve the lower back needs. The spine is exposed from the upper back to the pelvis, screws are placed at each level, and the construct is anchored to the pelvis to prevent failure at the base.
Osteotomies are performed where correction is needed, with larger bone cuts achieving the greatest realignment for severe deformity. Corrective maneuvers restore front-to-back and side-to-side balance under continuous nerve monitoring. Extensive bone grafting supports the long fusion, and the wound is closed over drains.
