What is posterior spinal fusion?

Posterior spinal fusion is the foundation of modern spine stabilization. It is performed from behind, placing pedicle screws into the strong bony pillars of the vertebrae and connecting them with rods to hold one or more segments in a corrected position while bone graft fuses them permanently.
It is the most common approach to spinal fusion and underlies the treatment of a wide range of conditions, including instability, slippage, deformity, and degenerative disease. CONSERVATIVE CARE FIRST

What we try before surgery.

Fusion is recommended only after appropriate non-surgical care has not resolved the problem, or when there is clear instability or deformity. Physical therapy, medication, activity changes, and injections are evaluated first.

When instability, slippage, or deformity continues to cause symptoms despite conservative care, posterior fusion provides durable stabilization.

THE PROCEDURE

How posterior fusion is performed.

The patient is positioned face down and the back of the spine is exposed at the levels to be fused. Pedicle screws are placed on both sides at each level under imaging or navigation guidance, providing strong three-dimensional control of the spine.

Where needed, the nerves are decompressed and interbody cages or osteotomies are added. Rods connect the screws and are contoured to restore alignment, and the surfaces are prepared and packed with bone graft so the segments fuse solidly over the following months. The wound is closed over a drain.
