A spinal osteotomy is a controlled bone cut that allows a rigid or severe deformity to be realigned, used when screws and rods alone cannot move the curve.

OVERVIEW

What is a spinal osteotomy?

A spinal osteotomy is a controlled bone cut that allows the spine to be realigned when a deformity is too rigid to correct with screws and rods alone. Osteotomies range from smaller cuts that gently open up a level to larger cuts that remove a wedge or an entire vertebra to achieve major correction.

Smaller osteotomies are used across several levels to gradually restore the natural curve. A pedicle subtraction osteotomy removes a wedge of bone to achieve thirty to forty degrees of correction at a single level, and a vertebral column resection removes an entire vertebra for the most severe, rigid deformity.

Osteotomies are usually part of a larger deformity correction or reconstruction. Dr. Enguidanos is fellowship trained in all osteotomy techniques under Dr. Thomas Lowe at the University of Colorado, and performs them at HCA Florida Twin Cities Hospital in Niceville, Florida.

What we try before surgery.

Osteotomy is part of correcting significant, rigid deformity that limits function. Conservative care is tried first for the underlying deformity where appropriate.

When a deformity is rigid and causing disability that conservative care cannot address, an osteotomy provides the correction that instrumentation alone cannot.

THE PROCEDURE

How an osteotomy is performed.

The spine is exposed from behind and screws are placed above and below the level to be cut. Continuous nerve monitoring is in place throughout. The planned bone cut is made precisely, whether a smaller opening cut, a wedge removal, or a full vertebral resection.

Once the cut is complete, the spine is carefully realigned through the osteotomy and held in the corrected position with rods. The correction is checked clinically and on imaging, bone graft is applied, and the wound is closed over drains. The level fuses in its new, corrected position.
