Posterior cervical fusion approaches the spine from behind, allowing decompression of several levels and stabilization with lateral mass screws and rods.
Posterior cervical fusion approaches the cervical spine from the back of the neck. It is used when several levels need to be decompressed, when there is cervical deformity to correct, or when the front-of-neck approach alone would not provide enough stability.
The procedure uses lateral mass screws placed in the bony elements of the cervical vertebrae, connected by rods, to stabilize and fuse the treated levels. A laminectomy is usually done at the same time to remove the structures pressing on the spinal cord and create more room.

Posterior cervical fusion is reserved for problems that cannot be managed without surgery, such as significant cord compression or instability. Where appropriate, physical therapy, medication, and injections are tried first.
When imaging shows multilevel compression or deformity that is causing neurological symptoms, surgery becomes the appropriate path to protect the spinal cord and restore stability.
The patient is positioned face down with the head secured and continuous spinal cord monitoring in place. A midline incision exposes the back of the cervical spine. Lateral mass screws are placed on both sides at each level to be fused, with careful planning to protect nearby arteries.
A laminectomy removes the bony plates pressing on the cord, allowing it to shift back into the new space. Rods connect the screws on each side, and bone graft is applied to support fusion. The wound is closed over a drain.
Posterior cervical fusion is one of several cervical approaches. The right choice depends on the number of levels, your alignment, and the cause of compression.
