OVERVIEW

What is regenerative medicine?

Regenerative medicine for the spine uses the patient’s own cells and growth factors to reduce inflammation, support tissue healing, and potentially slow the progression of degenerative changes. The two main options are stem cell therapy and platelet-rich plasma, or PRP.

Stem cell therapy uses cells taken from the patient’s own bone marrow or fat, injected into the affected disc, joint, or space around the nerves. These cells have anti-inflammatory properties and may support local repair. PRP is prepared from the patient’s own blood by concentrating the platelets and the growth factors they contain, then injecting them into the targeted structure.

Dr. Enguidanos offers these as additions to comprehensive spine care, not as replacements for surgery when surgery is clearly indicated. He gives an honest discussion of the current evidence and realistic expectations before recommending any regenerative treatment. CONSERVATIVE CARE FIRST

What we try before surgery.

Regenerative treatments are themselves a conservative, non-surgical option, used alongside physical therapy, medication, and activity changes. They are considered for appropriate patients before surgical intervention.

When structural deformity, progressive weakness, or significant instability is present, surgery rather than a biological injection is the appropriate path, and Dr. Enguidanos will say so directly.

THE PROCEDURE

How treatment is performed.

Not everyone is a good candidate. Dr. Enguidanos reviews imaging, symptom severity, and prior treatment to decide whether regenerative options are likely to help. For stem cell therapy, cells are taken from the patient’s own bone marrow or fat under local anesthesia and processed to concentrate them. For PRP, the patient’s blood is drawn and spun to concentrate the platelets, which takes about twenty to thirty minutes.

The prepared material is injected under imaging guidance into the targeted structure, the disc, a joint, or the space around the nerves, to ensure accurate delivery. These are outpatient procedures, and surgical options remain available if the response is not adequate.
