Revision spine surgery corrects or repairs prior operations that did not achieve the intended result. It begins with understanding exactly what failed, and why, before deciding what can be done.
Revision spine surgery is any procedure performed to correct a prior operation that did not achieve the intended result, or that caused a problem needing surgical correction. It covers hardware removal and replacement, extending a prior fusion, correcting deformity that developed after surgery, repairing a fusion that did not heal, and re-decompressing a nerve that was not fully freed.
Revision surgery is more demanding than first-time surgery for several reasons. Prior scarring distorts the normal anatomy, prior hardware occupies space around the nerves, bone quality may be reduced, and the original reason for failure must be identified and addressed rather than simply repeated.
For rigid or severe curves, osteotomies, controlled bone cuts, are added to allow adequate correction. Dr. Enguidanos is fellowship trained in all of these techniques under Dr. Thomas Lowe at the University of Colorado, the former president of the Scoliosis Research Society. Some symptoms after surgery are part of normal recovery and are managed without further surgery. Imaging and a careful history determine whether a correctable problem is present.
When imaging confirms hardware failure, a failed fusion, or ongoing nerve compression, and conservative measures have not helped, revision surgery is the appropriate path.
Every revision begins with a complete review of prior operative reports, implant records, imaging, and history. The approach is chosen based on what needs to be addressed, which may be from behind through prior scar, from the front through fresh tissue, or a staged combination.
Prior scar is carefully dissected to identify and protect nerves that may be stuck to scar tissue, the most demanding part of revision surgery. The specific cause of failure is addressed, hardware is removed or replaced, a failed fusion is repaired with new graft and fixation, or the construct is extended. New revision-grade implants are placed and the wound is closed over drains.
