Microdiscectomy removes the herniated disc fragment pressing on a nerve root through a small incision under the microscope, relieving sciatica while leaving the rest of the disc intact.
Most herniated discs improve on their own over six to twelve weeks as the body reabsorbs the fragment. Physical therapy, anti-inflammatory medication, activity changes, and epidural injections are tried first.
When leg pain persists beyond that window, or when there is significant weakness, microdiscectomy reliably relieves the nerve compression and is the appropriate next step.
The patient is positioned face down. A small incision is made over the affected level and a tubular retractor or small opening provides access under the microscope. A small window is made in the lamina to reach the nerve root.
The nerve root is gently protected and the herniated fragment pressing on it is removed. The rest of the disc is left in place. The nerve is confirmed free and mobile, and the small wound is closed. Most patients walk within hours and go home the same day.
Laminectomy is one of several ways to relieve lumbar nerve compression. The right choice depends on whether the spine is stable and what is causing the crowding.
